Mediform GmbH sells MediVoice, an AI telephone assistant that answers calls from patients, books, reschedules and cancels appointments, and prepares prescription requests for staff approval. Its MediChat product provides online appointment booking for a practice website. The company states that both products integrate with practice management systems such as Medical Office, Tomedo and T2Med.

Customers are physician practices and medical care centres (MVZ) in Germany, Austria and Switzerland; the website states that over 1,000 physicians use the product. Pricing is published as per-minute billing for MediVoice, a monthly fee per calendar for MediChat, and a combined package on request, with no minimum term.

Mediform was founded in 2022 as a spin-off of Innoopract, a software company founded in 1995, and is headquartered at Lammstraße 21 in Karlsruhe. The company states that patient data is hosted in certified data centres in Frankfurt and that its cloud infrastructure holds a BSI C5 attestation. At the time of capture, three open roles were listed in Karlsruhe: a working student for product and practice digitisation, a practice onboarding manager, and a head of marketing.
